Primary Care Networks
Primary Care Networks use a team-based approach to health care, centered around the clinic you visit and your family physician or nurse practitioner. We call this your “medical home”.
Our services are available only to the patients of physicians and nurse practitioners who are members or participating providers of our PCN, with the exception of some publicly available classes and workshops. SASPCN’s maternity clinic may also accept patients without a referral if they are good candidates for the program.
Our Team
Our healthcare team members – including nurses, dietitians, mental health nurses, pharmacists, and referral coordinators – work with physicians and nurse practitioners to support patients throughout their health journeys.
