Pharmacist Support

Taking medications can be overwhelming and confusing. Your medical home’s pharmacist can sit down with you or talk to you over the phone and answer any questions or concerns regarding your medications. Ask your family doctor if you are interested in a referral to a medical home pharmacist.

Some of the services a primary care pharmacist can provide include:

    • Medication reviews – this means reviewing all of the medications you are taking, including vitamins, herbs, supplements, and over-the-counter medications, as well as prescription drugs.
    • Drug information – this can include dose adjustments, drug therapy assessment, medication interaction checks, product availability, and drug funding advice.
    • Medication management – we can talk to you about your medications, including increasing benefits and minimizing side effects, if you are taking them for a chronic condition such as diabetes, chronic pain, high blood pressure, high cholesterol, asthma, heart failure, or chronic obstructive pulmonary disease (COPD).
    • Smoking Cessation – PCN pharmacists can help you with quitting smoking or vaping by providing information and resources, and by explaining the pharmaceutical options available to help.
    • Collaborative care – we can help if your care is taking place in multiple locations and/or with multiple providers, such as liaising with home care and other community partners, and providing hospital discharge support.
