Primary healthcare is essential for everyone, but for individuals 65 and over, a solid medical home for healthcare is particularly important as healthcare requirements tend to become more complex with age. When a patient is part of a primary care practice – with a consistent family doctor that you see regularly – they can see a variety of care providers and be assured that their information will be organized within the same system, ensuring continuity of care.

Our Services

Health and Care Coordination

As we age, we may face age-related issues like chronic disease, mobility problems, and more.

At SASPCN, our team includes physicians, nurses, behavioural health consultants (mental health nurses), pharmacists, dietitians, and referral management coordinators.

Our healthcare team can support you to maintain your health and prevent illness, with the aim of assisting patients and families to manage the challenges associated with ageing. In collaboration with the primary care provider, they can help you keep track of changes, set goals, and coordinate care with specialists.


Specialized Seniors Clinic

Seniors with complex medical issues can be referred to the Geriatric Evaluation and Management (GEM) clinic. This is an outpatient consultative interdisciplinary team who provide specialized, comprehensive assessment and management, working collaboratively with your family physician.

This service is available to seniors aged 65 and older who are referred by their SASPCN family physician.

Caregivers Alberta

Alberta’s only dedicated caregiver organization. They focus on helping caregivers maintain their wellbeing rather than teaching them how to be better caregivers.

St. Albert Seniors Association

SASA hosts a 50+ Seniors Activity and Services Centre in downtown St. Albert. There are programs for members and non-members, and they are the local Meals on Wheels Hub.
